"I Thomas Boutilier, aged 20 years occupation unemployed, do declare on oath that my personal description is: Color white, complexion fair, height 5 feet 7 inches weight 168 pounds, color of hair brown color of eyes grey other visible distinctive marks none. I was born in Sydney Canada on the 8 day of October, anno Domini 1907; I now reside at 668 Park Place Brooklyn, New York. I emigrated to the United States from Mulgrave Canada on the vessel Canadian National RR; my last foreign residence was Mulgrave Canada; I am not married;....I arrived at the port of Vanceboro in the State of Maine, on or about the 7 day of May, anno Domini 1925....[etc.]"

Obtained a small country cottage home in Dutchess County, New York in 1953. Obtained a suburban home in Hempstead, New York in November 1964 and moved away from Brooklyn permanently. Source-deeds for both properties in possession of the author.
cremated July 21, 1993, according to death certificate in possession of the author. Death certificate states that Thomas S. Boutilier died at the home of his son Michael Boutilier in Bronx, New York; informant was Michael Boutilier. Death registered in the Bronx, cert # CITE.
